All new recipients must have the following information:

1. Proof of address (i.e. recent Electric bill, Phone bill, etc.)

2. Proof of income (if any).

3. Names and ages of everyone living in household.

Fill out and bring it to Pike County H.E.L.P. before the next scheduled distribution day.
DO NOT BRING IT ON THE DAY OF FOOD DISTRIBUTION.

Be sure to bring a laundry basket(s) to the Center on distribution day. Those who do not have basket(s) on the day of distribution may have a longer wait.
